Extra virgin olive oil (EVOO) is 73% oleic acid — a monounsaturated fat with documented anti-inflammatory activity via NF-kB inhibition. It contains oleocanthal — a phenolic compound with ibuprofen-like COX-2 inhibiting activity (the anti-inflammatory bite at the back of the throat when consuming high-quality EVOO is the oleocanthal). It is the most studied dietary fat for cardiovascular protection, with the PREDIMED trial (7,447 patients) showing significant reduction in major cardiovascular events.
Available at every grocery store. Kirkland (Costco) EVOO is among the best value and authenticity-tested options. California Olive Ranch is a reliable brand available at most grocery stores.
Oleocanthal — only in high-quality extra virgin olive oil — inhibits COX-1 and COX-2 in the same mechanism as ibuprofen. The PREDIMED trial is the largest cardiovascular nutrition trial ever conducted: Mediterranean diet with EVOO (4+ tablespoons daily) reduced major cardiovascular events by 30% compared to low-fat control. For the APEX population with 55% hypertension and elevated cardiovascular mortality, EVOO at clinical doses is a direct intervention. The monounsaturated fat also improves insulin sensitivity and reduces LDL oxidation — the oxidized LDL that deposits in arteries.
Use as the primary cooking fat for all non-high-heat cooking. Drizzle on finished dishes. 4 tablespoons daily is the PREDIMED protocol dose. Use for sautéing greens, dressing salads, finishing soups, drizzling on grain bowls. HIGH HEAT CAUTION: EVOO's smoke point is 375-400°F — suitable for most cooking but not deep frying. For high-heat cooking, use avocado oil instead.
