Psyllium Husk

Psyllium husk (Plantago ovata seed husks) is the only fiber supplement with an FDA-approved health claim for cardiovascular disease risk reduction — and it is directly relevant to Black Americans who carry the highest cardiovascular mortality burden in the nation. The viscous gel-forming soluble fiber binds bile acids, traps glucose, and feeds Bacteroidetes species in the gut microbiome that are specifically depleted in T2D and hypertension. This is medicine that costs $0.15 per serving at Walmart.

Why it is ours

Metamucil and store-brand psyllium husk are available at every Walmart, CVS, Dollar General, and Family Dollar. This is the most financially accessible fiber intervention in the APEX pharmacopoeia — effective at $0.10-0.20 per serving regardless of income tier. Health education around psyllium is one of the highest-ROI interventions available for Black American cardiovascular health at scale.

What it does

Psyllium husk's arabinoxylan fiber forms a highly viscous gel in the intestinal lumen that operates through four simultaneous mechanisms: (1) Bile acid sequestration — the gel binds bile acids in the ileum, forcing the liver to synthesize new bile from cholesterol, reducing LDL by 5-15%; a 2018 meta-analysis in the American Journal of Clinical Nutrition (28 RCTs, 1924 participants) confirmed psyllium reduces LDL by 0.33 mmol/L (approximately 13 mg/dL); (2) Glucose trapping — arabinoxylan gel delays carbohydrate absorption, blunting postprandial glucose spikes with AUC reduction of 10-20% documented in T2D patients; (3) Blood pressure reduction — a British Medical Journal 2020 meta-analysis found dietary fiber supplementation reduced systolic blood pressure by 1.65 mmHg; (4) Prebiotic fermentation — psyllium arabinoxylan selectively feeds Bacteroides and Bifidobacterium species, producing short-chain fatty acids that reduce colonic inflammation and strengthen gut barrier integrity. Black American-specific relevance: Black Americans have documented gut microbiome dysbiosis patterns (reduced Bacteroidetes, elevated Proteobacteria) associated with higher dietary refined carbohydrate exposure — psyllium directly corrects this shift.

How it is cooked

1 tablespoon psyllium husk in 8-12 oz water, stirred immediately and drunk before it gels completely. Follow with an additional 8 oz water. Take 30 minutes before meals for maximum glucose-trapping effect. Can be added to smoothies, oatmeal, or yogurt. Start with 1 tsp and increase to 1 tablespoon over 1-2 weeks to avoid gas and bloating.
